<h2 class="wp-block-heading">Régénération cutanée : le principe commun</h2>



<p class="wp-block-paragraph">Avant d&rsquo;entrer dans les différences, il faut poser une base. PRP et exosomes appartiennent tous deux à la médecine régénérative, c&rsquo;est-à-dire qu&rsquo;ils visent à stimuler les mécanismes naturels de réparation de la peau.</p>



<h2 class="wp-block-heading">Le PRP : les facteurs de croissance de votre propre sang</h2>



<p class="wp-block-paragraph">Le PRP, ou <a href="https://www.forever-beauty.com/en/treatment/prp-face/" target="_blank" rel="noreferrer noopener">Plasma Riche en Plaquettes</a>, est fabriqué à partir d&rsquo;un prélèvement sanguin réalisé le jour même de la séance. Ce sang est centrifugé pour isoler le plasma concentré en plaquettes, qui contient une grande quantité de facteurs de croissance : PDGF, TGF-β, VEGF, entre autres. Ce concentré est ensuite réinjecté dans les zones à traiter : le visage, le cuir chevelu, le décolleté.</p>



<p class="wp-block-paragraph">L&rsquo;action du PRP est progressive et cumulative. Les plaquettes libèrent leurs facteurs de croissance sur plusieurs jours, ce qui stimule la production de collagène, améliore la vascularisation locale et accélère la réparation tissulaire. Le résultat visible demande généralement 4 à 6 semaines et se construit sur un protocole de 3 séances espacées d&rsquo;un mois.</p>



<p class="wp-block-paragraph">Un avantage que nos médecins apprécient particulièrement : le PRP est autologue. Il est préparé à partir du sang du patient lui-même, ce qui rend les réactions d&rsquo;intolérance rares.</p>

<h2 class="wp-block-heading">Les exosomes : la signalisation cellulaire de nouvelle génération</h2>



<p class="wp-block-paragraph">The <a href="https://www.forever-beauty.com/en/treatment/exosomes/" target="_blank" rel="noreferrer noopener">exosomes</a> sont des vésicules extracellulaires nanométriques, des petites capsules libérées naturellement par les cellules pour communiquer entre elles. En médecine esthétique, on utilise des exosomes issus de cellules souches d&rsquo;origine végétale, purifiés et concentrés, que l&rsquo;on applique sur la peau.</p>



<p class="wp-block-paragraph">Leur mode d&rsquo;action est différent du PRP : les exosomes ne contiennent pas de cellules vivantes, mais transportent des ARN messagers, des protéines de signalisation et des microARN qui influencent directement le comportement des cellules de la peau. En termes simples, ils donnent des instructions : produire plus de collagène, réduire l&rsquo;inflammation, accélérer la cicatrisation.</p>



<p class="wp-block-paragraph">En pratique, les exosomes s&rsquo;utilisent en application topique intensive après des traitements ablatifs (laser fractionné, microneedling, peeling profond). Leur capacité à pénétrer en profondeur et à potentialiser les effets d&rsquo;autres traitements est ce qui les distingue le plus nettement du PRP.</p>



<p class="wp-block-paragraph">C&rsquo;est aussi un domaine où la recherche avance vite. Since we added it to our range, along with the <a href="https://www.forever-beauty.com/en/treatment/cryolipolysis-coolsculpting/" target="_blank" rel="noreferrer noopener">CoolSculpting®</a>, We have treated thousands of patients at our clinic in Geneva. </p>



<p class="wp-block-paragraph">This volume of cases has taught us what clinical studies don't always tell us: why some patients achieve spectacular results, why others are disappointed, and how to guide therapeutic decisions with honesty.</p>

<h2 class="wp-block-heading">How does cryolipolysis work in practice?</h2>



<p class="wp-block-paragraph">CoolSculpting® is a non-invasive procedure that breaks down fat cells located directly under the skin by administering controlled cooling to the skin surface. The destroyed fat cells are then naturally assimilated by the body over a period of several months.</p>



<p class="wp-block-paragraph">Visible results appear on average one month after the session. The definitive result takes six months. This is a point that we systematically explain to our patients, as many expect to see a rapid transformation. Cryolipolysis doesn't work like that. It's a slow, biological reduction that takes place over time.</p>



<p class="wp-block-paragraph">In practice, one session produces a reduction of around 30 % in the volume of fat in the treated area. If the bulge is more than 3 cm thick, we plan two sessions spaced two months apart to achieve sufficient results.</p>



<p class="wp-block-paragraph">The treatment does not replace conventional methods such as diet, exercise or liposuction. It is a complement for resistant localised areas, not a global weight loss solution.</p>



<h2 class="wp-block-heading">Situations where cryolipolysis gives the best results</h2>



<p class="wp-block-paragraph">After 25 years of practice, we have identified an ideal candidate profile: a person with one or two localised bulges, resistant to sport and diet, on an overall stable body. The best documented areas are the abdomen, flanks, thighs, arms, submental and submandibular region, back, subgluteal folds, arms and bra area.</p>



<p class="wp-block-paragraph">This treatment is not suitable for overall weight loss. That is not its role. Patients who arrive with a targeted figure and realistic expectations are the ones who leave satisfied. Those who hope for a spectacular all-round effect are invariably disappointed.</p>

<h2 class="wp-block-heading">What we have learned about side effects</h2>



<p class="wp-block-paragraph">Transient side effects are frequent and expected. They occur in the treatment area during the session and in the days that follow.</p>



<p class="wp-block-paragraph">During the session: sensation of pressure, tightness, intense cold, tingling, cramp - these sensations diminish as the area becomes numb. Immediately afterwards: redness, stiffness, temporary whitening. Over the following one to two weeks: bruising, swelling, itching, numbness that may last several weeks.</p>



<p class="wp-block-paragraph">Two rare effects deserve particular attention.</p>



<p class="wp-block-paragraph">L’<strong>paradoxical adipose hyperplasia</strong> is manifested by an increase in the volume of fat in the treated area, visible between 2 and 5 months after treatment. Its frequency is estimated at around 1 in 3,000 treatments (0.033 %). When it occurs, surgery - liposuction - may be necessary.</p>



<p class="wp-block-paragraph">La <strong>demarcation of the treatment area</strong> (approximately 1 treatment in 20,000, i.e. 0.005 %) corresponds to a visible depression in the treated area, the result of excessive localised fat removal.</p>



<p class="wp-block-paragraph">Very rare effects have also been documented: frostbite (1 in 15,000), hyperpigmentation (1 in 11,000), vagal symptoms such as dizziness or fainting (1 in 30,000), cold panniculitis (1 in 60,000). These data are taken from the CoolSculpting® safety form (Allergan Aesthetics, CH-CSC-210028, November 2021).</p>



<p class="wp-block-paragraph">These risks are documented in the informed consent form signed by each Forever Institut patient. This is not an administrative formality: it is a guarantee that the decision has been taken with full knowledge of the facts.</p>

<h2 class="wp-block-heading">Contraindications that we strictly apply</h2>



<p class="wp-block-paragraph">The safety of the treatment depends on a rigorous assessment before each session. Certain situations constitute absolute contraindications: the treatment must not be carried out.</p>

<p class="wp-block-paragraph">Other situations do not constitute absolute contraindications, but require a thorough medical assessment before proceeding with treatment:</p>



<ul class="wp-block-list">
<li>Known sensitivity to cold: cold urticaria, Raynaud's disease, frostbite or frostbite</li>



<li>Reduced peripheral blood circulation in the area to be treated</li>



<li>Neuralgic pain: post-herpetic neuralgia, diabetic neuropathy</li>



<li>Bleeding diseases or taking anticoagulants</li>



<li>Recent surgery or scar tissue in the area to be treated</li>



<li>Hernia in or near the treatment area</li>



<li>Active skin disorders: eczema, dermatitis, rashes on the skin area</li>

<h2 class="wp-block-heading">Why do results vary from one patient to another?</h2>



<p class="wp-block-paragraph">This is the question we hear most often after a treatment. Two patients, same apparent morphology, same area treated: one is delighted, the other expected more. There are many reasons for this.</p>



<p class="wp-block-paragraph">Firstly, the composition of the bulge. Some bulges contain more fibrous tissue than pure adipocytes, which reduces the response to cold. Lifestyle also plays a direct role: the effects observed persist over time only if the patient maintains a balanced diet and regular physical activity. Otherwise, the remaining fat cells can compensate for the loss of destroyed cells.</p>



<p class="wp-block-paragraph">Cryolipolysis reduces the number of adipocytes in a given area. It does not alter overall metabolism or eating habits. This point, repeated at each follow-up consultation, is the key to lasting satisfaction.</p>



<h2 class="wp-block-heading">Our medical view of growing demand</h2>



<p class="wp-block-paragraph">In the space of just a few years, cryolipolysis has gone from being a confidential treatment to a common request, fuelled by social networks and sometimes misleading before-and-after images. We are seeing an increase in misdirected requests: patients who wish to treat unsuitable areas, or who downplay the medical information needed to speed up treatment.</p>

<p class="wp-block-paragraph">Behind this visibility lies a real technology: the <a href="https://www.forever-beauty.com/en/treatment/led-experience/">photobiomodulation</a>, Its clinical effects have been documented since the 1990s. But the difference between a device selling for CHF 60 online and a <a href="https://www.forever-beauty.com/en/treatment/led-experience/" target="_blank" rel="noreferrer noopener">LED treatment</a> performed under medical supervision, the resemblance ends with the shape of the mask. This guide explains why.</p>



<p class="wp-block-paragraph"><strong>What LED therapy really is</strong></p>



<p class="wp-block-paragraph">Photobiomodulation (or LED light therapy) is based on a simple principle: certain wavelengths of light penetrate skin tissue and trigger targeted biological reactions. Red light (630-670 nm) stimulates collagen production and accelerates cell healing. Blue light (415-430 nm) acts on the bacteria responsible for acne by disrupting their metabolism. Infrared light (760-850 nm) penetrates deeper to reduce inflammation and promote tissue regeneration.</p>



<p class="wp-block-paragraph">These effects are real. They are documented in the medical literature, reproduced in controlled studies, and have been used for years in dermatology and aesthetic medicine protocols. So the question is not «Does LED work?» but «Under what conditions does it really work?»</p>



<p class="wp-block-paragraph"><strong>Why does power change everything?</strong></p>



<p class="wp-block-paragraph">A consumer LED light is subject to strict safety standards that limit its irradiance - i.e. the density of power emitted per square centimetre of skin. These limits exist so that anyone can use the device at home without risk, without prior assessment, without a health professional nearby. This is a legitimate protection strategy. But it has a direct cost in terms of effectiveness.</p>



<p class="wp-block-paragraph">Medical equipment used in institutes is not subject to the same restrictions. Their irradiance can be adjusted according to phototype, the area treated and the clinical indication. The «dose» of light delivered (measured in joules per square centimetre) is calculated to be therapeutically active, not simply harmless.</p>



<p class="wp-block-paragraph">In concrete terms: a consumer mask typically delivers between 10 and 30 mW/cm². A medical device can deliver between 100 and 200 mW/cm², or even more depending on the protocol. For the same length of session, the energy received by the skin is not comparable.</p>

<h2 class="wp-block-heading"><strong>Wavelengths: precision vs. approximation</strong></h2>



<p class="wp-block-paragraph">A good quality LED mask will display its wavelengths on the product sheet. A cheap LED mask will also display its wavelengths, but the accuracy of the actual emission may vary by 20 to 40 nm from the advertised value. For anti-acne blue light, a deviation of 30 nm is enough to take you outside the bactericidal action window. For red light, inaccuracy reduces the depth of penetration and the activation of fibroblasts.</p>

<p class="wp-block-paragraph"><strong>Medical supervision: what the device does not replace</strong></p>

<p class="wp-block-paragraph">Let's be frank: a consumer LED mask is not without effect. On healthy skin with a compatible phototype and with regular use, the red light can improve radiance, slightly reduce signs of fatigue and support the skin barrier. For someone who doesn't have regular access to a medical institute, it's a maintenance supplement that has its place.</p>



<p class="wp-block-paragraph">But it does not treat. It does not correct severe acne-prone skin. It does not stimulate deep collagen synthesis. It does not replace a post-procedure protocol. And for darker phototypes (V and VI), some consumer devices present risks of irritation if the blue wavelength is incorrectly calibrated.</p>



<p class="wp-block-paragraph">The main limit is not technological, but biological: below a certain irradiance threshold, the cellular response is not activated. You can use your mask every day for three months and still remain below this threshold.</p>

<h2 class="wp-block-heading">How does a laser remove a tattoo?</h2>



<p class="wp-block-paragraph">The principle is based on the selectivity of laser light. The beam emits very short, very powerful pulses that target the ink particles encysted in the dermis. Under the effect of heat, these particles fragment into micro-particles that the immune system can then eliminate naturally.</p>



<p class="wp-block-paragraph">Each session reduces the concentration of ink in the skin. The discolouration is progressive: the tattoo fades with each session, until it disappears or becomes imperceptible. The interval between two sessions - generally six to eight weeks - gives the body time to evacuate the ink fragments and the skin time to regenerate.</p>

<h2 class="wp-block-heading">How many sessions on average?</h2>



<p class="wp-block-paragraph">The usual range is between 5 and 15 sessions, depending on the case. According to the doctors at Forever Institut, a small amateur tattoo in black ink on light skin can be removed in 4 to 6 sessions. On the other hand, a large polychrome tattoo done by a professional using dense coloured inks may require 12 to 15 sessions, sometimes more.</p>



<p class="wp-block-paragraph">The interval between two sessions is 6 to 8 weeks. This period is not an arbitrary constraint: it corresponds to the time needed for the body to evacuate the fragments of ink produced during the previous session and for the skin to regenerate completely.</p>

<h3 class="wp-block-heading">1. The colour of the ink</h3>



<p class="wp-block-paragraph">This is the most important factor. Black ink absorbs all wavelengths of light, so it responds most quickly and completely to lasers. Red, orange and yellow inks are more selective: they require specific wavelengths and respond less well to standard lasers.</p>



<p class="wp-block-paragraph">Green and blue inks are the most resistant. Some pigment formulations used in professional studios contain compounds that absorb laser light poorly, multiplying the number of sessions required.</p>



<h3 class="wp-block-heading">2. The size and complexity of the tattoo</h3>



<p class="wp-block-paragraph">The size of the tattoo is not related to the effectiveness of tattoo removal. The density of the line does, however, play a role: a very heavy tattoo, with saturated areas of fill, will require more sessions than a tattoo with a fine line. The quality of the ink will also play an important role in the effectiveness of tattoo removal: inferior quality ink will take longer to remove.</p>



<h3 class="wp-block-heading">3. Age of the tattoo</h3>



<p class="wp-block-paragraph">Counter-intuitively, an old tattoo is not always harder to remove. Over time, the body has already begun to naturally fragment some of the ink. A recent tattoo, less than a year old, has ink that is not yet well stabilised in the dermis and so may respond better to laser treatment.</p>



<p class="wp-block-paragraph">In both cases, the protocol can be adapted. A tattoo that is more than ten years old can sometimes be treated in fewer sessions than one that is five years old, simply because some of the pigmentation has already faded.</p>



<h3 class="wp-block-heading">4. Quality and type of ink</h3>



<p class="wp-block-paragraph">Tattoos made by amateurs or with low-quality inks are often easier to remove. The ink is generally less dense, deposited less deeply in the dermis, and more homogeneous. A professional studio tattoo, on the other hand, uses concentrated, multiple inks deposited with precision - which gives a superior aesthetic result, but makes tattoo removal more complex.</p>

<p class="wp-block-paragraph">On fair skins (phototypes I to III), the contrast between the ink and the skin is maximal. The laser can work at higher fluences, which improves effectiveness at each session. On dark or matt skins (phototypes IV to VI), it is important to adjust the wavelength and to treat the tattoo with Nd:YAG to avoid any risk of hyperpigmentation.&nbsp;</p>

<p class="wp-block-paragraph">Blood and lymph circulation in the treated area influences the speed at which ink fragments are eliminated. <p class="wp-block-paragraph">Laser hair removal is a medical technology that uses a monochromatic beam of light, i.e. a single wavelength, to precisely target the melanin in the hair follicle. This highly concentrated light is absorbed by the hair pigment, transformed into heat and then permanently destroys the root without damaging the surrounding tissue.</p>

Moving? No. A metamorphosis. A page turned as if opening a new book of skin, listening and light. </strong></p>



<p class="wp-block-paragraph"><strong>Dr Polla, anchoring change<br></strong>Forever is first and foremost the mark of one man: Dr Polla, a visionary pioneer who ushered in a new era in aesthetic medicine, one of precision, naturalness and innovation. His institute was never simply a place for treatment: it was a whole philosophy! An art of gesture that whispers rather than imposes. An approach where the human element always comes before the technical. This soul, this signature, survives the move intact. Forever keeps its heart, even if its walls change.</p>

Once injected, it forms a natural fibrin matrix that gradually releases the growth factors, prolonging collagen stimulation and cell regeneration over time.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> What is EZ Gel and how is it prepared?</p>



<p class="wp-block-paragraph"><strong>Dr. Wahlen :</strong> EZ Gel is an autologous gel, i.e. obtained from the patient's own blood. Two tubes are taken: one is heated to transform the albumin into a gel, and the other is used to recover the PRF. By mixing them, we obtain a gel-like texture containing the regenerating factors of the PRF. This gel acts as a natural light volumiser and, above all, regenerates the skin. The regenerative action is longer-lasting than that of FRP alone, thanks to the enveloping and protective action of the gel. However, the volumising effect does not last as long as with an industrial hyaluronic acid filler.</p>

<p class="wp-block-paragraph"><strong>Journalist :</strong> Why is this treatment so popular in the field of beauty and skin regeneration?</p>



<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> Because it's part of a major trend: natural regeneration. More and more patients are looking for visible but subtle results, without resorting to fillers. PRF and EZ Gel optimise the skin's natural potential, encouraging regeneration rather than correction. The treatment is safe, with no allergic risk or overcorrection effect, and can often be suitable even for those who cannot benefit from other techniques.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> What are the main benefits for the skin?</p>



<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> They improve the overall quality of the dermis: radiance, density, firmness and elasticity. PRF and EZ Gel reduce fine lines, restore volume, lighten dark circles and help tissue repair. They can also help to improve certain hyperpigmentation or light scars. As with other biostimulation treatments, you need to have reasonable expectations and patience to see how effective the treatment is over time.</p>

<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> PRF and EZ Gel are particularly good for the delicate area around the eyes. I like to combine the two treatments in several layers for optimum regenerative action. They can also be used all over the face, and even on the scalp and hands.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> Is it a preventive or corrective anti-ageing treatment?</p>



<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> Both. For young skin, it's an excellent preventive treatment that maintains skin quality and revives radiance. For more mature skin, it stimulates the repair and collagen production processes.</p>

<p class="wp-block-paragraph"><strong>Journalist :</strong> How many sessions are needed to see results?</p>



<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> In general, two to three sessions spaced four to six weeks apart are recommended, followed by a maintenance session once or twice a year. The response varies from person to person, as it all depends on the skin's natural capacity for regeneration.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> Can it be combined with other treatments?</p>



<p class="wp-block-paragraph"><strong>Dr. Wahlen :</strong> Yes, FRP works very well with skin boosters, biostimulants such as Radiesse, Sculptra or polynucleotides, as well as certain peels. It enhances their effects by improving the quality of the dermis.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> Are there any contraindications or side effects?</p>



<p class="wp-block-paragraph"><strong>Dr. Wahlen :</strong> The procedure should be avoided in the case of coagulation disorders, certain autoimmune diseases, anticoagulants, active infection or pregnancy. Side effects are slight: slight swelling may occur for 1 to 3 days in general, but as with all injections there is a risk of haematoma.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> Is the treatment painful? Is there any social exclusion?</p>



<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> The injections, which are never pleasant, are not usually painful. Some people resume their activities immediately, while others may experience slight swelling for 24 to 72 hours.</p>

<p class="wp-block-paragraph"><strong><strong>Dr. Wahlen :</strong></strong> Rather, it is an additional treatment available. PRP remains a recognised treatment. The choice always depends on the indication and the patient's profile.</p>



<p class="wp-block-paragraph"><strong>Journalist :</strong> What role do you see for FRP and EZgel in the future?</p>

<p class="wp-block-paragraph">An intimate conversation with a doctor.&nbsp;</p>



<p class="wp-block-paragraph"><strong>Your career path resembles a moult: from general medicine to gynaecology, then on to intimate aesthetics. How did this path take shape, almost as a matter of course?<br></strong>I started out as a general practitioner, and little by little a lot of patients came to me for gynaecological follow-up. Fitting IUDs, contraceptive implants, pregnancies... it all came naturally. As a woman and a doctor, I quickly felt that meeting these needs filled a gap. Then a laboratory contacted me and said: "You already practise gynaecology, so why not train in its aesthetic dimension? I agreed, without any taboos!&nbsp;</p>



<p class="wp-block-paragraph"><strong>When we talk about intimate health, we're talking about confidence and modesty, but also beauty and well-being. Was it natural for you to combine skincare, beauty and self-confidence?<br></strong>Yes, because everything is intimately linked. Vaginal dryness, for example, strikes many women after cancer or heavy treatment. All too often, they still hear: "There's no solution, you just have to learn to live with it". But restoring comfort and self-confidence is already a cure. Two out of three patients come to see me after having been badly treated, either by illness or by medical silence. That's when aesthetic gynaecology comes into its own: a deeply human response that goes far beyond appearance.</p>



<p class="wp-block-paragraph"><strong>Behind the closed doors of your practice, what are the most common reasons why your patients come to you?</strong>Vaginal dryness is the main cause, often linked to cancer treatments or the menopause. But there are also functional problems: pain when playing sport, discomfort when wearing tight clothes, or a loss of confidence in intimacy. Some come for labia majora reshaping, others to get their sex life back on track after years of hiatus. It's never "on a whim": it's a need for comfort and dignity.</p>



<p class="wp-block-paragraph"><strong>Today, techniques are multiplying... How do you choose the right gesture, the right note, to re-tune the intimate?<br></strong>There are several approaches. Hyaluronic acid injections, which rehydrate and restore. Radiofrequency, which improves vaginal suppleness and treats vaginismus. More recently, biophotomodulation uses light to stimulate collagen and balance intimate flora. Each protocol is chosen according to the patient's history. It's never standardised, always personalised.</p>



<p class="wp-block-paragraph"><strong>You often talk about singular stories. Do you have an anecdote that illustrates the alchemy between medical care and personal rebirth?<br></strong>A 68-year-old patient, widowed for almost two decades, had found the impetus of a new partner. Eighteen years without intimacy had left their mark, to the extent that a first intercourse caused her to tear her vagina. We worked on rehydrating the tissues, but also on relearning the language of attention: I took the time to talk with her partner, to evoke the tenderness of foreplay, the importance of delicacy. This dialogue proved to be as therapeutic as the medical procedure itself. Because, as I often say, in these cases, communication between the couple is an integral part of the healing process.</p>



<p class="wp-block-paragraph"><strong>In this month of Pink October, your arrival at Forever Institut is symbolic. What message would you like to send to women who are still reluctant to cross the threshold of an office to talk about intimacy?<br></strong>Dare to talk about it! There's nothing taboo about intimate health: it deserves the same vigilance as your heart, skin or eyes. Find a doctor with whom you feel confident, and confide your questions to him or her, even if your voice trembles when you put them down on paper. Cosmetic gynaecology is not a whim: it's a medicine for regaining comfort, sewing up self-esteem and reinventing relationships.&nbsp;</p>
